Who handles your information

This notice covers the GastroConsult website and its appointment enquiry form, operated for Dr Henry Wei. The Privacy Act 2020 and Health Information Privacy Code apply to the handling of personal and health information. The clinics where consultations and procedures take place also keep their own records and have their own privacy arrangements.

When you send an enquiry

The form collects the details you enter, such as your name, contact details, preferred location and the reason for your enquiry. Required fields are marked on the form. You choose what to include in optional fields. Without enough contact information, we may be unable to respond or arrange an appointment.

We use your enquiry to contact you, direct it to the appropriate practice and help organise your care. Dr Wei and the staff handling the enquiry may need to read it. Relevant information may be shared with the practice you select or the team arranging your appointment. Sending an enquiry does not confirm a booking.

Please provide only the information needed for your enquiry. Contact the relevant practice to arrange transfer of referral documents or detailed medical records. The form is for routine enquiries and is not monitored as an emergency service.

Storage, service providers and sharing

The website and enquiry form use Wix. Form submissions are stored in Wix, and enquiry notifications and replies use email services. These providers may process or store information outside New Zealand. You can read Wix’s privacy policy for information about its services and international processing.

Information is used and shared for the purpose for which it was collected, with your authorisation, or where the law permits or requires it. We do not sell information supplied through the enquiry form or use it to build advertising audiences.

Enquiries are retained for the time needed to respond, administer appointments and meet applicable record-keeping requirements. Information that becomes part of a clinical record is subject to the relevant health-record requirements. Deleting browser preferences does not delete an enquiry or a clinical record.

Cookies, browser preferences and website use

Wix and other services needed to deliver the website may receive technical information such as your IP address, browser details, pages requested and security logs. Cookies may be used to operate and protect Wix services, including the enquiry form. The form opens on a separate Wix address.

Our saved-guide, optional recent-guide, reading-size, motion and game-score features use storage on your own browser. These preferences are not sent to GastroConsult by those features and are not automatically shared between devices. Someone using the same browser may be able to see saved or recent guides.

You can clear GastroConsult’s saved guides, reading history, preferences and game scores here, or use your browser settings to clear site data. This does not clear data on the separate Wix enquiry-form address.

We have not added advertising pixels or session-recording software to this frontend. Links to other websites, the separate IBS tool and external map services have their own privacy practices.

Access, corrections and concerns

You can ask for access to personal information held about you and ask for it to be corrected. We may need to confirm your identity before releasing information. If information is not corrected as requested, you can ask for a statement of the correction sought to be attached to the record.

Changes to this notice

We update this notice when the website or its information-handling arrangements change. The date above identifies the latest version.
